In some cases, the third party (the person, conference or council you are allowing to complete the form) does not have an internet connection or sufficient technology to complete the form online.
You can then print the form so that it can be completed manually or download it to send by email. A PDF and a Word version of each form are available.
These forms should not be sent directly to the CGI: You, as the national delegate, will then need to create your online application using the information completed manually by your third-party declarant (Conference or lower-level Council).
